We can see you at home or in a care home.  Our aim is to maximise your independence.

We take a holistic approach with our assessments and will help to signpost and refer to other agencies, professionals as / if needed.

  • Generalised weakness following hospital admission.

  • Advice on equipment, for example walking aids and splints
